Outpatient Pharmacy Services

Retail Pharmacy

The Lima Memorial Outpatient Pharmacy is conveniently located on the second floor of the hospital. Our team of pharmacists and technicians are available to fill your prescriptions and provide education about your medications before you leave the hospital. The Retail Pharmacy is open Monday to Friday from 7 a.m. - 5 p.m.

Meds-to-Beds

We believe the first and only stop after you leave the hospital should be your home. Our Meds-to-Beds service ensures your prescriptions are filled and ready for you when you leave the hospital. This option is available for all patients leaving the hospital, including patients discharging from the Emergency Center and outpatient surgery. Most major prescription insurance plans are welcomed.

Emergency Center

A pharmacy technician is available 24/7/365 to ensure updated medication records, reviewing and documenting your current prescriptions, supplements and allergies.

Anticoagulation Clinic & Diabetic Services

Our pharmacists are available Monday - Friday, 7 a.m. - 4:30 p.m., in our anticoagulation (Coumadin) clinic for anticoagulation management / therapy, such as stroke prevention, minimizing bleeding risks, and managing clotting disorders and anticoagulants (medications). Our pharmacists are also available Monday – Friday 7 a.m. – 4:30 p.m., in our diabetic center to provide diabetes management and education services. Our diabetic center not only offers education and support, we are also able to connect you to a team of experts who all work together to help manage your condition and symptoms. Inpatient Pharmacy Services

For patients admitted to Lima Memorial, our pharmacists are available at all hours to ensure you receive medications ordered by your provider. Our pharmacists work with you on your care plan, facilitate discharge processes and provide you with education prior to your discharge from Lima Memorial. Main Pharmacy

The main inpatient pharmacy at our hospital serves as the central hub for preparing and dispensing medications 24/7/365. Staffed with expert pharmacists and technicians, it ensures the safe, timely and precise distribution of medications to meet your unique needs throughout your stay.

Decentralized Clinical Pharmacists

Decentralized clinical pharmacists are integral members of our health care team and are stationed across various floors to provide specialized medication expertise directly at the patient's bedside. They collaborate closely with other members of our interdisciplinary team to optimize medication therapy, conduct antimicrobial stewardship programs, oversee discharge medication reconciliations and ensure the safest and most effective treatment plans tailored to your needs.

Pharmacy Residency Program

Great Care starts with providing great experience to the future of health care, and the Lima Memorial Pharmacy Residency Program is no exception. Established in 2015, the Lima Memorial Pharmacy Residency Program was first accredited by the American Society of Health-System Pharmacists in 2016. The purpose of the pharmacy residency is to build on PharmD education and expand clinical skills. This one-year program offers comprehensive and dynamic educational experiences across various patient care settings.

The residency program is characterized by its longitudinal rotations encompassing administration, anticoagulation clinic, diabetes management, drug information, leadership, operations, research and participation in the Teaching and Learning Certificate (TLC) Program at Ohio Northern University. The core learning experiences provide residents with a solid foundation in cardiology, critical care, infectious diseases, internal medicine and outpatient infusion. Additionally, residents can tailor their experience through elective learning experiences, including academia, ambulatory care, informatics, oncology and more.
