Cardinal Health Partners, Limited was established in 1996 as a Physician Hospital Organization (PHO) between Care Ohio, Inc. and Lima Memorial Hospital. Care Ohio, Inc. is the Physician Organization and equal partner with Lima Memorial Hospital in Cardinal Health Partners, Limited. Care Ohio assumes the credentialing responsibility for the PHO and Lima Memorial Hospital assumes the responsibility of negotiating contracts and communicating provider information to the Health Plans for the PHO. Periodic reporting to the health plans is shared by both partners as are all other obligations of the agreements between the PHO and Health Plans in our commitment to comply with NCQA standards and guidelines. There are more than 340 practitioners participating in the Cardinal network covering primarily Allen County and the contiguous counties. Each partner governs itself independently with separate Boards of Directors and certain members of Management of Lima Memorial Hospital and Lima Memorial Hospital Board.

CMS is requiring Health Plans to update the Provider information in their directories. We’ve been told in order to accomplish this, if each provider would update their CAQH profile and select Universal then any Health Plan you are contracted with will be able to access your CAQH information and obtain some of the data they need.
